HUM Rideshare is proud to offer the following Bodily Injury and Property Damage coverage under the Ride Assurance package of benefits offered through Fair American Select Company (Fairmatic).

Ride Assurance

HUM Rideshare is proud to offer the following Occupational Accident coverage under the Ride Assurance package of benefits.

A1) Occupational Accident Insurance CALIFORNIA ONLY:  

Accidental Death: Death Benefit:

• Three or More Total Dependents: $320,000

• Two Total Dependents: $290,000

• One Total Dependent and One or More Partial Dependents:

$290,000

• One Total Dependent and Zero Partial Dependents: $250,000

• Zero Total Dependents and One or More Partial Dependents:

an amount equal to 8 times the amount annually devoted to

the support of partial dependents, but not more than

$250,000.

Burial Benefit: $10,000.

These limits reflect Sections 4701 & 4702 of the CA Labor Code.

These limits will change in accordance with any changes in the

Labor Code.

Accidental Dismemberment: $200,000 Maximum Benefit

365 Day Commencement Period.

Accident Medical Expense: $1,000,000 Per Person/Accident.

$0 Per Accident Deductible.

90 Day Commencement Period

104 Week Payment Period.

Pre-Existing Conditions: No Exclusion or Sub-Limit

Temporary Total Disability: 66.66% of Average Weekly Earnings, subject to the following:

Minimum Amount per Week: {amount(s) set by California’s Division

of Workers’ Compensation for the year/years the Certificate is issued,

pursuant to the Division’s authority granted by subdivision (a) of

section 4453 of the California Labor Code}

Maximum Amount per Week: {amount(s) set by California’s Division

of Workers’ Compensation for the year/years the Certificate is issued,

pursuant to the Division’s authority granted by subdivision (a) of

section 4453 of the California Labor Code}

Continuous Total Disability: 70% of Average Weekly Earnings up to

$400 Maximum Weekly Benefit minus Other Income Benefits.

Maximum Payment Period is

12 Months or to Age 70.

Combined Single Limit: $1,000,000 Per Person.

Combined Single Limit applies to all

coverages except Temporary Total Disability (TTD).

Benefits paid under TTD do not erode the Combined Single Limit.

Coverage Periods:

a. beginning when a Covered Person turns the Policyholder’s platform on and is available to accept

rides (Period 1)*;

b. continuing when a Covered Person accepts a request through the Policyholder’s platform, and is

on route to the first requested pick-up location. (Period 2);

c. continuing while the Covered Person transports person(s) for hire; and ending when the

person(s) have been delivered and the Covered Person is no longer engaged and available to

accept rides (Period 3).

* Period 1 will be prorated if more than one TNC or DNC Platform is engaged, and the Covered Person is

not engaged in Periods 2 or 3 with another TNC Platform. If there is another TNC or DNC platform

being engaged by the Covered Person in Periods 2 or 3, then the engaged Platform will be Primary, and

We will be Excess. If the person is on a personal deviation and engaged in personal activities, there is no

coverage.
